Wellness Associates is an out of network provider. This means we do not accept any insurance. We bill patients directly at the time of service. While we are an out of network provider, you may be eligible to receive reimbursement through your insurance company. After payment is received, you will be provided with documentation, which can be submitted to your insurance company for reimbursement.

We do not directly communicate with your insurance company. Please contact your insurance provider to inquire if you have out-of-network mental health/behavioral health benefits.

If you have out-of-network mental health benefits, the following additional questions may be helpful to ask your insurance company:

  • Is preauthorization required for mental health/behavioral health services?
  • What is my annual deductible for mental health services?
  • Have I met my annual deductible for this year?
  • Does my plan cover a limited amount of sessions per calendar year?
  • Am I responsible for a co-insurance payment, and if so, what is the amount? 
  • What is the typical rate for outpatient psychotherapy in my area (CPT code 90834)?
